Child Care In-Service Training Certificates

For personnel working in DCFS-licensed child care programs, all conference workshops qualify toward fulfillment of your annual in-service training requirement of 15 clock-hours per year. To document the clock-hours earned at this conference, use the Certificate of Attendance at the back of this program. Complete one line for each workshop attended. Take it to your center director for approval or keep it on file in your licensed home. Note that CPDU forms (described at the top of this page) are not needed in child care centers, except for certified teachers who have submitted Certificate Renewal Plans.
